We’ll meet face-to-face to talk about your needs and desires as well as your worries and concerns. We’ll gather information from you along with any family members or close friends you want involved to help you determine current and future goals, concerns or obstacles.
Personalized Written Assessment
Stacey LaFlamme, the owner of Dovetail Advocacy, has 20 years of hands-on experience working in hospitals and home health care. She will research and suggest supports that can be put in place to meet your goals. She will advise you on various options available to you and create a personalized written plan with recommendations for your immediate and future needs that can be implemented whenever you choose.
Care Coordination & Management
Dovetail Advocacy will work with you to coordinate and manage your care services. We can make introductions, handle setup and scheduling, and be your one contact for all your services and supports to help simplify the process for you.
Ongoing Support & Advocacy
Dovetail Advocacy is located in Wisconsin’s Lake Country, and is able to remain hands-on throughout, advocating for your needs and adjusting your care plan as needed.
